Submittal Process

Make your payment using the payment link or mail a check to 7114 W Jefferson Ave Ste 301, Lakewood CO 80235.
Once your payment has been made please submit form below on this page.
You must provide the full name on the credit card used to pay & merchant number found on your paypal receipt.
Once we match the payment to the submitted form we will email confirmation that your request has been received. This can take a day or two.
