Payment Plan Request Form

    Use this form to request up to an 18 month payment plan arrangement. Payment shall be in equal monthly amounts provided that the monthly payments are a minimum of $25 per month. There is a ​$5 per month administrative fee to set up and monitor the payment plan over the chosen months. You may choose fewer than 18 months. Once your request has been reviewed and approved we will be notified in writing. By submitting this form you are accepting the payment plan. An Owner who has entered into a repayment plan may elect to pay the remaining balance owed under the repayment plan at any time during the duration of the repayment plan. ​



    By submitting this form I acknowledge that, if I fail to pay at least 3 of the monthly installments within 15 days after the monthly installment is due, this payment plan will become null and void and my account will be turned over to the Association Attorney for collections.
